IF THIS IS YOUR FIRST APPOINTMENT PLEASE REMEMBER TO BRING ALONG THE FOLLOWING:
✓ A referral letter from your GP or treating specialist (required for a Medicare Rebate)
A referral from your GP will be valid for 12 months from your initial visit.
A referral from a specialist will be valid for 3 months from your initial visit.
Please email a copy to our rooms if you are able to do so reception@southsydneymedicalspecialists.com.au.
✓ Your Medicare card and any concession card
✓ Your medication list
✓ Any relevant investigation such as blood test, scans and x-rays
✓ Any previous medical correspondence related to your medical condition
✓ Your blood sugar record book and glucometer if you are a pre-existing diabetic. If you have recently diagnosed with diabetes, DO NOT WORRY! We will guide you through this process.
We recognise that your time is valuable, and we make every effort to run on time. Occasionally emergencies occur or patients require a little more time. These cause scheduling delays beyond our control and we sincerely apologise in the event we keep you waiting.
➤ Please arrive at least 10 minutes prior to your appointment time to allow time to complete your patient registrations forms.

We have on site pathology service and accept any referral forms
Consultation Fees
At the time you make your appointment, you will be advised of the consultation fees and how much you can get back from Medicare.
Unfortunately we are not a bulk billing practice and all consultations will attract a fee.
We require full prepayment for all telehealth appointments and our reception team will call you 24 hours before your appointment to confirm your appointment and take payment.
Cancellation Policy
Should you need to cancel your appointment, please telephone our office during business hours (please do not leave a message) and allow at least 2 day's notice so that we can offer your appointment time to the many patients on our waiting list. Otherwise a cancellation fee may be charged.
